Geoff Schackmann’s journey in healthcare leadership is marked by a clear focus on innovation and efficiency. With his roots in Computer Science from the University of
Oregon, Geoff has harnessed technology to streamline operations and enhance patient care across several home health and hospice startups. Yet, his decision to enter the space is marked by a personal connection to caring for the frail and elderly. Geoff took a keen interest in the space when his mother became ill and required extensive home health services.

leadership, and business growth has been pivotal. Geoff has been known to marry his background in computer science with his love of the home health and hospice space, by
introducing his own technologies to streamline operations, augment patient and clinician experience, and ensure all of his companies are an employer of choice.
From 2015 to 2020, Geoff, as a co-owner, brought his strategic vision to the healthcareentities under Healthy Living at Home, expanding services throughout Oregon, Washington, and Arizona. He built operations that combined traditional care with cutting-edge approaches, all within the stringent Joint Commission’s quality standards. The company was divested in 2020 where Geoff was given the opportunity to take his skill sets from the home health industry and apply them to the hospice industry.
In 2021, he co-founded VistaRiver, expanding its reach and developing its services in the Pacific Northwest. Geoff’s role in Oregon has been central to the company’s progress and compliance with healthcare regulations. Under Geoff’s leadership, he has led local agencies, spearheaded organic and de novo growth opportunities, and implemented key standard operating procedures and consumer experience programs – including the Perfect Visit; a patient centric framework on how to deliver care.
His latest venture, h/care, co-founded in 2023 with Ryan Haller, reflects Geoff’s ongoingcommitment to advancement. This initiative intertwines generative AI with in hometechnology, reshaping the home health and hospice landscape. The business has amicrofocus on taking the most common points of dissatisfaction carried by clinicians andeliminating those to grow key performance indicators such Medicare star ratings, clinician turnover, census growth, and gross margin. In his role at h/care, Geoff serves as the Founding Principal with a focus in all operation and tech matters.

With nearly two decades of trailblazing leadership in healthcare finance, M&A, real estate development, and project management, Ryan Haller’s career has been built on the adage “think differently.” His career is distinguished by a relentless pursuit of innovation, particularly within the post-acute healthcare sectors of senior living, skilled nursing, home health, and hospice services.
Ryan’s professional journey commenced at Deutsche Bahn, Europe’s premier transportation provider, where he orchestrated the quality M&A integration of BAX Global, a €3.2 billion venture. His strategic leadership over five years united 85 offices across the Americas. Ryan also played a role in laying out the Sarbanes Oxley program within BAX.

In 2010, Ryan’s first foray into start-ups was his co-founding of Promontory Healthcare, an early adopter in moving expensive post-acute care out of a hospital setting and into standalone transitional care centers across America. His expertise in market analysis, financial underwriting, and real estate development laid the groundwork for the company’s growth, as he led the full cycle of concept to delivery for an array of TCUs in the Midwest and Pacific Northwest.
Ryan moved on to work with Fortress Investment Group (NYSE: FIG) and Newcastle REIT overseeing the Holiday Retirement portfolio. During Ryan’s tenure, he managed a team of 30 professionals to integrate over $1 billion in acquired senior housing assets, reporting directly to the CEO. He was also instrumental in developing Holiday Retirement’s Assisted Living platform as the Chair of the Steering Committee, emphasizing his foresight in senior care evolution.
As the President and Chief Development Officer for Avamere Group, Ryan’s vision gave rise to his next start-up, PDCo, enhancing the group’s trajectory in seniors housing, home health, and hospice services. Under his guidance, Avamere grew its portfolio size from 45 to 67 communities, growing all the way to $1 billion in revenue and 10,000 employees. His founding of the Ovation brand was instrumental through his studying under the leadership of the Ritz Carlton Leadership Academy. He also played a critical role in growing Signature Home Health and Hospice from a two-state to a five-state regional leader in at-home care. He successfully fundraised in excess of $300 million of capital during his tenure.
In 2019, Ryan launched the WELL Companies (“WELL”), ushering in a new era of senior living with several innovative brands such as Varcity, America’s first brand to build intergenerational housing in college campuses. This pioneering venture sought to redefine housing for Baby Boomers, culminating in significant media attention from the New York Times and Daily Beast. Ryan was also an invited speaker at NASDAQ, discussing innovative ways society can break down ageism through housing. Later that year, Ryan was an invited guest speaker at the Vatican to address the importance of intergenerational relationships between the oldest and youngest generations. Ryan created rare joint ventures with top firms in America such as Delos, the International Well Building Institute, and Mission Wealth to carry out WELL’s ethos. In 2023, Ryan completed the sale of the firm to an NFL family.
Currently, Ryan’s entrepreneurial spirit lives on through h/care, a venture he co-founded in 2023 with Geoff Schackmann. This latest endeavor marries generative AI with in-home technology to revolutionize home health and hospice care. Positioned as a high-touch, high-tech, high-trust deliverer of home-based care, h/care aims to address the inefficiencies plaguing the industry, aligning with Ryan’s enduring commitment to improve care delivery and experience for older adults. h/care marks Ryan’s third career venture in the home health and hospice space, a critical element to the future of how Boomers will age.
Ryan has established himself as a visionary in entrepreneurship, with his engaging lectures on business creation, eldercare, and development strategies drawing audiences at prominent universities across the country. His commitment extends to serving as a revered guest speaker at various universities each year. Beyond lecturing, Ryan actively participates in the Young Presidents Organization (“YPO”) Bluegrass Chapter and is the pioneering founder and Forum Moderator of YPO’s international home health and hospice forum. Ryan is a native Nebraskan, a lifelong learner, an effusive believer in better, a tenacious problem solver, and a seamster for efficiency in all of his business endeavors.

With nearly three decades of leadership and success in finance, entrepreneurship, and strategic consulting, Tim has built a distinguished career on the foundation of “creative problem-solving.” His extensive experience spans not just investments and the financial markets, but also real estate, advisory work, and community service, reflecting his commitment to innovation and excellence.
Tim is the Founder and Managing Director of Lone Eagle Capital, a firm focused on underwriting and sourcing compelling investment opportunities, and committing capital, strategic advisory, and/or consulting services to help drive the success of those investments. Lone Eagle Capital has worked with hedge funds, private equity, real estate, and venture capital. Tim’s vision and leadership have driven Lone Eagle Capital to success, supporting early-stage growth companies and providing strategic consulting and advisory services.

Tim is also a Senior Managing Director at CAVU Securities, LLC. CAVU Securities is a veteran and minority-owned institutional broker-dealer. CAVU offers a wide range of products and services across cash management, capital markets, and the advisory landscape. CAVU’s focus is to provide partners with the ability to access best-in-class products and services, when possible within the construct of DEI support and participation. As a part of CAVU’s social impact objectives, the firm commits financial support to nonprofit partners via its Impact Pledge—which is 10% of gross revenue on capital market deal fees and the CAVU money market fund share classes.
As a consultant and business development expert, Tim has made significant contributions to firms such as BlueCrest Capital Management, Invictus Capital Management, and Fay Financial, among others. His strategic insights on the business and investment front, and ability to secure substantial capital allocations, have proven invaluable in driving growth and profitability.
Tim’s community involvement is equally impressive. When he was based in Incline Village, he served as a Deputy Constable, a food delivery assistant with Project MANA, and a mentor with the Big Brother/Big Sister program. His commitment to education is evident in his roles as a former guest lecturer at Sierra Nevada College and a volunteer teacher at the Lake Tahoe School.
In Texas, Tim continues to make an impact through his involvement in institutional finance, entrepreneurial ventures, and youth education and athletics. Tim has coached youth football and baseball for over 7 years and has helped support educational institutions, especially those assisting children with special needs. As a 7th generation transplanted Texan, Tim is a member of the Sons of the Republic of Texas and the Former Texas Rangers Association. Tim’s deep roots and dedication to his community are a testament to his character.
Tim holds a B.S.E. in Chemical Engineering and a certificate in Environmental Studies from Princeton University, where he graduated in 1994. He is also an alumnus of St. Mark’s School in Southborough, MA.
